چکیده

Cases are admitted from time to time to most large hospitals in a state of coma or semi-coma in which the differential diagnosis includes overdosage with the barbiturate series of narcotic drugs. It is often important to confirm or exclude this factor as quickly as possible. Standard methods of analysis (e.g., the Stas-Otto process, Smith, 1934a), although ultimately giving accurate quantitative results, are time-consuming. An endeavour has been made to evolve a more rapid method of analysis which is sufficiently delicate to give an answer to the question of whether -or not barbiturate poisoning plays a significant part in the clinical condition of the patient. The method in our hands takes about three hours to perform and is based upon the facts that (1) the barbiturate drugs, being derived from barbituric acid, are either the acid derivatives, e.g., diethylbarbituric acid (barbitone), or their sodium salts, e.g., sodium diethylbarbiturate (soluble barbitone). (2) When in aqueous solution they can be changed from one form to the other by making the pH acid or alkaline. (3) The barbituric acid derivatives are only slightly soluble in water but are readily soluble in ether, whereas their sodium salts are readily soluble in water and almost insoluble in ether. As part of any ingested barbituric acid is excreted unchanged in the urine, at least 100 ml. of urine are collected and examined in all cases. Unless the state of coma is known to have lasted for a long time, the gastric contents are aspirated and examined as well.

A rapid method of identifying particular barbiturate derivatives in small samples of blood by paper chromatography.

The quantitative estimation of barbiturates in blood by ultra-violet spectrophotometry (Goldbaum, 1948; Walker, Fisher, and McHugh, 1948; Born, 1949; Lous, 1950; Wright and Johns, 1953) is of limited value unless the particular barbiturate derivative is known or can be identified. Goldbaum (1952) described an ultra-violet spectrophotometric technique by which he claimed to make such identificat...

Fluorometric screening method for detecting benzodiazepines in blood and urine.

We report a fluorometric method for detecting diazepam, chlordiazepoxide, oxazepam, chlorazepate, and (or) their major metabolites in blood, urine, or gastric contents at low therapeutic concentrations. The drugs are first hydrolyzed to their respective benzophenones and converted to highly fluorescent 9-acridanones. Total benzodiazepines (parent plus metabolites) in blood and gastric contents ...

Biochemical Markers of Acute and Chronic Ethanol Use: Forensic and Clinical Applications

Background: The analysis of ethanol in bio-samples is an important method for determination of acute alcohol use/abuse both in clinical and forensic toxicology. In forensic cases, it is known that micro-organisms involved in the postmortem putrefaction process can produce alcohol and when the body has been traumatized. In clinical setting, post-ingestion time has a critical role for determinati...

In vitro anti-proliferative activity of clove extract on human gastric carcinoma

Background and objectives: Cancer cell resistance to common chemotherapy agents is on rise. Plants are considered valuable sources of herbal drugs for cancer therapy. The present study was conducted to investigate the in vitro antioxidant, anti-proliferative, and apoptosis-inducing properties of clove (Syzygium aromaticum L.) extract in human gastric carcinoma...

Correlation between Cigarette Smoking and Urine Cotinine Level in Gastric Cancer Patients

Various substances in cigarette smoke including nicotine have been shown to promote/induce cancer cell proliferation. Since cotinine has a longer half life and stability in the blood, it has become the preferred biomarker for cigarette smoking exposure. Seventy-three gastric cancer patients were included in this study. The tumor tissues were stained with H and E for pathological evaluation. The...
